It was requiring a Costco number from me, because I left it blank, filled out the rest of the info and the next page said you left out the following information and it said the information needed was the Costco number.
I apologize. They must have changed this because I requested my sample without any problem last night. I am a Costco member but I didn’t have my card handy and left that field blank and it still went through. Thanks for letting us know!
